What is Osgood-Schlatter Disease?

Osgood-Schlatter Disease is a condition primarily observed in young athletes, characterised by the presence of a painful and swollen lump situated just below the knee joint. This condition is often seen in adolescents and is associated with activities that impose significant stress on the lower extremities, such as running, jumping, or participating in sports that involve repetitive high-impact movements.

The affected area spans from the patella tendon, which connects the kneecap (patella) to the shin bone (tibia), to the tibial tuberosity, which is a prominent bony protrusion located at the top of the shin bone. During demanding physical activities, the patella tendon experiences repetitive traction or pulling forces, particularly at its insertion point on the tibial tuberosity.

The continuous and excessive traction force exerted on this point of tendon insertion can lead to inflammation and irritation of the immature bone of the tibial tuberosity. This inflammatory response results in localised pain, swelling, and tenderness, giving rise to the characteristic lump or prominence just below the knee joint.

Individuals affected by Osgood-Schlatter Disease typically experience discomfort and pain during or after engaging in activities that stress the knee joint, especially those involving running, jumping, or sudden changes in direction. The pain may be exacerbated by activities that require bending or straightening of the knee, such as squatting or climbing stairs.

While Osgood-Schlatter Disease is generally self-limiting and tends to resolve with time as the individual’s bones mature and strengthen, management focuses on alleviating symptoms and promoting optimal healing. This may involve rest from aggravating activities, application of ice packs to reduce inflammation, use of nonsteroidal anti-inflammatory drugs (NSAIDs) for pain relief, and physical therapy to improve strength, flexibility, and biomechanics of the lower extremities.

Most individuals with Osgood-Schlatter Disease can expect gradual improvement and resolution of symptoms over time with appropriate management and activity modification.
